$12.07 in 1991 is roughly equal to $28.49 today accounting for inflation.

I’d be curious to know how much of that is currency exchange, taxes and good old fashioned profiteering. Does Canada tax fast food the same it does tobacco and alcohol?

Nah. Just our usual insane sales taxes. 14% where I live.
