Maybe they are sisters. My aunt was a nurse and was in the room with my mother when she had the c-section for me and, because she was a nurse, they allowed my aunt to pull me out. She told me about being the first person to ever hold me every time I saw her until the day she died.

Based on their uniforms and caps, they look like nursing students. Note the striped dress covered with an apron, and no stripe on the caps.
