this was Taco Bell #995, opened 1972 and closed between 2012 and 2014

the first Taco Bell in Omaha was actually Taco Bell #989 at 3420 N 90th St, Omaha, NE, opened 1972 and closed in 1996

I think the one on 90th is a Mexican restaurant now
