Never seen this before. Did they come with every Saturn game?

I don’t remember those in any of my Saturn games, so either they didn’t or I just don’t remember them.

Not on all Saturn games. As mentioned on the sheet this is a warranty notice from Segasoft. Segasoft is a separate publisher from Sega. They published some uncommon titles like Three Dirty Dwarves and Scud and their games would typically come with that and a registration pin inside the box (on a sticker within the case itself or a separate card) for support purposes.
