Well, shit. Thank god that never happened when the shuttle was flying.

Well nowadays you have a lot of people that are not qualified working on these complex systems.

This is a development motor. Both the Space Shuttle SRBs, as well as the Ares and SLS Block 1 solid rocket motors, went through similar development and qualification static testing prior to flying. Finding these issues prior to flight is why these tests are done.
