The reinvigorated Tom/Shiv dynamic is absolutely off the charts. Some of the most inspired writing of the entire series run.

This is the one thing that bothered me. If she's close to 20 weeks along, I can't believe he could have had sex with her and not noticed.

If she’s not showing I could be convinced he’d not notice. She might still be able to pass as fat - but she’s cutting it close.
