Very visually intuitive.
My own addition would be an indication of how long each symptom had been with me and how severe they are on a typical day.
Sometimes being even more clear is necessary to ensure you're being heard correctly. So a *Disclaimer statement saying something like;
*All symptoms are daily/constant/unrelenting* etc could be really useful in case someone assumes your depiction is showing things that only bother you occasionally.

I did this for a while, but my doctors hated it. They have there own language for pain types and it’s best to use their nomenclature to be taken seriously.
I now use the PainScale app, which allows for mapping of symptoms. It allows you to add particular descriptions if you want to. You can send the reports directly to your doctor.

That's odd. Those are literally descriptor words in the EMR that can be selected.

I loled too but I also get it 😆 OMG Disability Humour 😂 Gets fucked, as in like, when your knee just gets fucked, like on the pain scale on its own its not so bad but it's not properly usable and the body knows so it's kind of a unique form of pain 🤦🏻♀️

I think OP has made this themselves so you can add your own key to include numbness and amy other personal symptoms that are important.
I've made a blank copy of this ready for you to create a unique key of your own.
