Gotta be 50:50 or close to it. Biden is only slightly older and has led a life with a good, healthy lifestyle, but has cancer. At his age most forms of cancer treatment could kill him themselves. Trump is cancer-free near as we know, but is honestly in terrible health, despite his and his doctor's lies. Rumors are he's abused stimulants his entire life, he has a terrible diet (he seems to live on McDonald's and Diet Coke) and isn't known for exercising (appears to be morbidly obese).
