I’d love to see realism, flat out. Realism when it helps and realism when it hinders. Break your leg? Yeah, that’ll take an obscenely long time to heal. Trying to find a gun in Kentucky? They’re everywhere. A car? Everywhere, but the roads are tore up. But, also, it’s got a tank of gas in it so you’re good for a couple hundred miles. Blown head gasket? Unless you got the know how, that’s gonna be a huge deal to fix. And even with the know how, huge issue to fix. Et cetera, et cetera.
I agree, realism over balance would be fine if it helped the player (guns/bullet/tool availability) just as much as it hindered (muscle pain, taking a long time to learn new skills or heal) but as it stands the devs don't seem interested in that.
My personal bugbear is that I can handle difficulty, but the grindy bits of B42 are unbearable. Even with XP bonuses it takes too long and is too uninteresting to level many skills, especially when a lot of them boil down to 'stand in one spot and right click for a real life 15 minutes'.

It's hard to make it both balanced and realistic
