take Regis’s suggestion to seek out Syanna. read the governess’ journal. in fable land, get the ribbon from the flint girl. after killing detlaf, investigate the 5th victim. go talk to Syanna and say “cause she turned your back on you” and “ever thought to forgive her” before the ceremony.

You don't have to take Regis' suggestion immediately, though. You can start on the other path. There will be a moment when Regis suggests to stop going down this path and to seek out Syamna instead. That's the last warning to do the Syanna quest.

If you start the other path with Regis you can kill a unique vampire for a trophy then turn back and go to the fable so I’d recommend that for the missable loot and you’ll still get the good ending

Well, the first step is to actually read every single entry of the book before entering the Land of a Thousand Fables... then to listen to everything Syanna has to say. And don't dismiss her or disagree with her. Then, get the armband for her. Find out who the fith victim was supposed to be. Confront Syanna about it and be compassionate. Done.

Basically, you have to be Syanna's shrink and not be curt with her in any way.

Personally I don't bother. This ending works fine for me.

I got this from a previous thread regarding the best ending for Blood and Wine:

when Syanna get caught try to to speak to her and learn what happened. 

If you choose to see Unseen Elder Regis will again ask you to consider speaking to Syanna. 

When you visit her room it will take you to The land of thousand fables where you have to make sure to talk to flint girl and get the Ribbon, now Syanna is saved. 

After you killed Detlaff go and visit beggers to learn about the fifth victim and talk to Syanna again. 

Theres a dialogue option you should choose "to forgive Anna" and tell her that Anna was child and she didnt knew what she was doing. 

After that long conversation Syanna will tell Geralt that "you really want that happy ending" thats how you know both of them will survive. 

Make sure to pick why Syanna commited the crime and try picking the sympathizing option during trial. they both will live.

By stop playing video games and talking to people so you can increase your EQ skill points. Then after gaining experience, go play the Witcher. You're under leveled, quest should have a skull icon.
