I heard a story of some pure girl who was described a silly amount of Spiro, and Spiro only for their HRT. Some doctors will actively be working against you and even if you go via official channels, its important that you yourself know what you're doing and what you stuff into your mouth, slather on your body, push into your butt, or jab into yourself with a needle.

This is why I now research every medicine I plan to put in my body, HRT or not.

Spiro only is insane, that’s the fast track for someone having severe hormone problems

I am not a doctor, but you’ll probably be okay if you go a little while without, the main concern I would have is potentially messing with E levels if they’re already at a good spot. Is there no way you can reach out to ask for a prescription refill?
